How to Create feedback forms and questionnaires

Collecting feedback is an essential part of any successful medical conference, workshop, or CME event. With BookCPD’s built-in Feedback module, organizers can quickly create customized feedback forms for delegates, and faculty  — all in just a few simple steps.

BookCPD allows you to create powerful feedback forms for your events. You can create multiple feedback forms targeted at either delegates or faculty. While feedback forms are most commonly used for post-event evaluation, they can also be used for other purposes such as a pre-course experience survey.


Combining Feedback Forms with Certificates

Feedback forms can be integrated with certificates. When combined, delegates receive an email with a link to download their certificate. However, they are first shown the feedback form and must complete it before the certificate is released. This is particularly useful for post-event evaluation.

Creating a Feedback Form

Go to the Feedback Section

Go to your Event Dashboard and click on the Feedback button. This takes you to the Feedback section. 



Click Create Feedback to load the feedback builder.

Enter Feedback Form Details

Complete the following details in the feedback builder:

  • Feedback Name — a name for your own identification, not displayed to delegates, e.g. Delegate Feedback 1
  • Mandatory — select whether delegates are required to open and submit the feedback form before accessing their certificate. Note that individual questions can still be left unanswered unless set as required within the question itself.
  • Feedback Title — this is the title that will be displayed to delegates
  • Audience — select whether this form is for delegates or faculty

Click Next at the bottom of the screen to proceed to the feedback designer.

Review the Pre-Populated Questions

When the feedback designer loads, a pop-up message will explain that a set of commonly used questions has already been pre-populated for you. These sample questions include:

  • Two questions about the delegate
  • A standard lecture or workshop feedback section using a radio grid
  • A final section about the course overall

You can modify, copy, or delete any of these questions as needed.

Build and Organise Your Questions

The feedback designer is feature-rich and similar to familiar tools like Google Forms or SurveyMonkey. It supports the following question types:

  • Single line text
  • Long text / multi-line paragraph
  • Radio buttons
  • Checkboxes
  • Dropdown boxes
  • Star rating
  • Scale slider
  • Matrix scale (commonly used for lecture feedback)

On the right-hand side panel you will find additional settings for each question, allowing you to change the properties of individual questions. Most commonly used settings — such as making a question mandatory — can be set directly within the question itself.

You can drag and reorder questions, and also move them between pages. Breaking a long form into multiple pages improves the experience for delegates, particularly on mobile.

Customise the Matrix Scale Question

Some question types require the right-hand panel for advanced editing. This is the most advanced feature in the designer, but straightforward once you know where to look.

The pre-populated lecture or workshop feedback question uses the Single Select Matrix type, with ratings from 1 to 5 across three rows: Content, Presentation, and Learning Objectives.

To remove the Learning Objectives row and keep only Content and Presentation:

  1. Click on the matrix question
  2. On the right-hand panel, locate the section showing horizontal and vertical line settings
  3. Click on the horizontal lines option — this will display the row labels: Content, Presentation, and Learning Objectives
  4. Delete the Learning Objectives row

The same right-hand panel can be used to adjust advanced settings for any other question type.


Before You Finalise

Preview and Save

It is important to preview the feedback form before issuing it to delegates. Click the Preview button at the top of the designer to see exactly how the form will appear to delegates, including the title, question order, and layout across all pages.

Once you are happy with the questionnaire, click Save.

ⓘ The feedback form can be edited again at any time after saving. Simply return to the Feedback section in your Event Dashboard, locate the form, and click Edit to make any changes.
